    Chapter 1 From the Patient Case History of SusanRichards Dr. Tamara Watkins, Ph.D, ABPP, APA Tuesday 16 th September, 2014 (Transcribed from voice recording) The time is 3:15 PM and the date is Tuesday the 16 th of September, 2014. This recording is for the case history of a new patient, Susan Richards, case number 2014-133-TW. Dr Susan Richards M.D. is a 33-year-old woman who lives with her husband and two young children. She presented to me for a routine psychiatric evaluation after a surgical incident resulted in the near loss of life of one of her patients. The hospital Morbidity and Mortality Board of Review found no fault on the part of any of the surgical staff, and after consultation with Dr Richards, I believe she harbours no guilt over the incident and see no indicators of resultant anxiety or depression. In my opinion, the incident has not impacted upon Dr Richards’ ability as a doctor and surgeon; I recommend that she continues her work uninterrupted. Dr Richards also complains of irregular periods of heightened anxiety (not aligned to her menstrual cycle) resulting in a sense of reduced well-being, which she claims is chronic and unrelated to the abovementioned incident. Although she has previously treated the anxiety though the use of prescription pharmaceuticals, she expresses a desire to explore an ongoing therapeutic treatment through counselling. ~~~ This is an interesting case.     out, there may be an interesting Case Study in it. Freud has often been discredited and lambasted for maintaining that all neuroses are rooted in sex. I have a little plaster bust of ‘The Father of Psychoanalysis’ in my consulting room, so it’s gratifying to at least occasionally come upon a case where he turns out to be right! Let’s start with the woman herself. Susan is a 33-year-old Anglo-European woman. She has a petite build – about 5’3” and 115lb – and since it may bear on a case involving sex and self-image, I would describe her as strikingly beautiful. She has blonde styled hair cut just beyond shoulder length, dainty features, and a classically elegant sense of personal grooming. Susan is a bright and engaging conversationalist with a sharp wit and a playful sense of self-deprecation, which she used to advantage to alleviate the potentially awkward first meeting with a psychiatrist. She articulated herself clearly using clinical language familiar to medical professionals and answered questions spontaneously without displaying any symptoms of the anxiety of which she complains.
